Welcome to The Midwest Sinus Center from our doctors and our professional support staff. We appreciate the opportunity to serve you and will do all we can to make your visit a pleasant experience. This newsletter will give you some insight into our practice philosophy and general information about our practice, our staff and policies.

Specialty and Services:
Our medical practice is limited to Otolaryngology - the surgical subspecialty that covers the diagnosis, surgical and medical treatment of diseases of the ear, nose and throat. Some of our services include:

    Our Physicians:
    The Midwest Sinus Center is a professional corporation of medical doctors (M.D.'s) who are trained in the surgical sub-specialty of Otolaryngology.

    William R. Panje, M.D., F.A.C.S. completed his premedical education at Iowa Wesleyan College in 1965. He went on to earn his medical degree and two Masters degrees at the University of Iowa by 1977. Dr. Panje completed his surgical internship, residency and post-graduate fellowship at the University of Iowa. Dr. Panje is double board certified in Otolaryngology - Head and Neck Surgery and Facial Plastic Surgery. He is presently the Director of the Head and Neck and Skull Base Surgery program at Rush Medical College where he also holds a full professorship. Dr. Panje has been on staff at Rush since 1992 and is the President-elect of the American Rhinologic Society.

    Neal M. Lofchy, M.D., F.R.C.S.C. completed his premedical education at the University of Western Ontario and the University of British Columbia. Dr. Lofchy earned his medical degree from the University of Toronto. He completed his surgical residency at both the University of California Los Angeles and the University of Toronto and completed a post-graduate fellowship under the direction of Dr. Bumsted at Rush-Presbyterian-St. Luke's Medical Center in 1994. Dr. Lofchy is board certified in Otolaryngology-Head and Neck Surgery in both Canada and the United States. He has been on staff at Rush since 1995.

    Jay M.Dutton, M.D., F.A.C.S. completed his premedical education at Indiana University in 1990 and earned his medical degree from University of Iowa in 1994. Dr. Dutton completed his surgical internship at Butterworth Hospital in Grand Rapids, Michigan and his residency training in Otolaryngology - Head and Neck Surgery at the University of Iowa in 1999. He then completed a post-graduate fellowship in Advanced Rhinology and Facial Plastic Surgery under the direction of Dr. Robert Bumsted at Rush Medical College in 2000. He is board certified in Otolaryngology - Head and Neck Surgery.

    Joseph P.Allegretti, M.D. completed his medical education at the University of Illinois - Urbana in 1986 and his medical degree from Loyola University of Chicago in 1990. Dr. Allegretti completed his surgical internship and residency at Indiana University in Indianapolis in 1995. He has completed three years of post-graduate fellowship training in Laryngoscopy, Head and Neck, Skull base, Facial Plastic, Reconstructive, Rhinologic and Sinus Surgery at both Rush Medical College and the University of Tennessee - Menphis. He is board certified in Otolaryngology and Head and Neck Surgery.

    How to contact our physicians:
    Your doctor may be at the hospital, in surgery or in one of our three offices, but our trained support staff will direct your call or message to him or the appropriate personnel.

    IF YOUR CALL IS AN EMERGENCY, PLEASE STATE SO IMMEDIATELY.

    After normal office hours, your call will be forwarded and answered by our answering service. In the event of an emergency, your message will be forwarded to the physician on call who will call you back as soon as possible.

    Appointments:
    Unless is it an emergency, we see patients by appointment only. Appointments may be made by calling the office number of the location nearest you. With three offices to serve you, the appointment desk will try to arrange your visit at a time and at an office most convenient for you. As a courtesy to us, if you are unable to keep an appointment, please let us know as soon as possible so that another patient may utilize the appointment time.

    You do not have to be referred by another doctor to make an appointment. In fact, many of our patients are sent to us by others we have cared for in the past. However, some pre-paid health plans require that you be referred by your assigned primary care physician to qualify for reimbursement. Please check with your respective plan before scheduling an appointment with one of our phsyicians.

    We understand that the relationship between you and your physician is very important. If for any reason you are not completely satisfied with the physician you have chosen, please let us know. We will make every effort to help you select a physician who may be more compatible with your needs.

    Prescriptions:
    Prescriptions are dated and marked as to the number of refills possible - this is done to protect the quality of our patient care. The pharmacist will not refill prescriptions over one year old. Please do not ask that we renew such prescriptions without proper evaluation of the need and effectiveness.

    Medications may have side effects. Should you have a reaction to drugs prescribed for you, STOP the drug and contact our office. ROUTINE PRESCRIPTIONS ARE NOT CONSIDERED AN EMERGENCY AND WILL NOT BE REFILLED AFTER NORMAL OFFICE HOURS.

    Surgery, x-rays and laboratory studies: The Midwest Sinus Center is dedicated to the concept of keeping you well and out of the hospital. Many procedures can be completed in our office or in an ancillary outpatient facility using the latest technology available.

    Certain specimens are sent to laboratories and pathologists outside of our practice. If your physician performs a biopsy or orders certain laboratory studies you may receive a bill from them and should direct your questions to that facility rather than to The Midwest Sinus Center.

    We attend to patients and perform surgery at Rush-Presbyterian-St. Luke's Medical Center, Good Samaritan Hospital , Ingalls Same Day Surgery Center and Midwest Center for Day Surgery.

    Your preference and convenience will always be taken into consideration when scheduling hospital stays and surgeries, however, sometimes timing and insurance plan coverage dictate location in the end.

    Fees and Insurance:
    Fees are payable at the time of your office visit unless other arrangements are made in advance of your examination. All co-pays are collected at the time of service. We accept MasterCard/Visa and personal checks for office visits.

    Medicare claims are filed by our office. We also participate in several managed care plans and preferred provider plans. If you are not a medicare beneficiary or member of a plan we participate with, your paid receipt will provide you with a detailed statement of services, charges and diagnosis so that you may file a claim with your carrier for personal reimbursement.

    If you require surgery, our insurance manager will give you a call to discuss your insurance coverage with you in advance of your procedure. A deposit may be required prior to your procedure. Our trained staff members are available to answer any questions you may have concerning your insurance coverage or our fee schedule. Please do not hesitate to discuss your concerns with us.
